Care Coordinator II

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Assist in care management activities and collaborate with designated member teams to ensure effective service delivery by providers/partners, maintain continuity of care, and enhance member satisfaction.

• Perform outreach to members through phone calls and home visits.

• Engage with members regarding care plans/service plans, discuss next steps, available resources, address questions and concerns, and provide ongoing education.

• Coordinate care-related activities with healthcare and community providers, partners, members, and caregivers.

• Address member and provider inquiries, requests, and concerns pertaining to care plans/service plans.

• Liaise with care managers, practitioners, and other stakeholders to promote member services and ensure continuity of care.

• Assist with service assessments/screenings and document the care needs of members.

• Record and maintain member documentation in accordance with state and regulatory standards.

• Recognize member needs and make appropriate referrals to Care Managers, community-based organizations, and Disease Managers.

• Educate members about available benefits and resources.

• Carry out additional responsibilities as assigned and adhere to all policies and standards.

• Must reside in Kansas.

• Proficient in technology.

• Strong communication and customer service abilities.

• Experience with outbound telephonic communication.

• Collaborative team player.

• Self-motivated individual.

• Effective problem-solving capabilities.

• Ability to adapt to changing environments.

• High school diploma or GED is mandatory.

• Required 1–2 years of related experience.
