This is a fully remote position, open to applicants in Arizona.
• Assist in care management tasks and support assigned teams to ensure the delivery of services and achieve continuity of care/member satisfaction.
• Conduct member outreach through phone calls or home visits.
• Document care and service plans along with related activities.
• Engage with members to discuss care plans, next steps, available resources, inquiries, concerns, and ongoing education.
• Coordinate care activities with healthcare providers, community partners, members, and caregivers.
• Address inquiries, requests, and concerns from members and providers related to care and service plans.
• Communicate with care managers, practitioners, and others to facilitate member services and ensure continuity of care.
• Assist in service assessments and screenings, documenting member care needs.
• Maintain member records in compliance with state and regulatory standards.
• Adhere to contractual, regulatory, and organizational standards and policies.
• Identify member needs and refer them to Care Managers, community-based organizations, and Disease Managers.
• Provide education regarding available benefits and resources.
• Carry out additional duties as assigned.
• A High School diploma or GED is mandatory.
• 1–2 years of relevant experience is required.
• Capability to conduct telephonic and home-visit outreach to members.
• Proficiency in documenting care/service plans and member records.
• Ability to collaborate with healthcare providers, community organizations, members, caregivers, care managers, and practitioners.
• Skill in identifying member needs and making suitable referrals.
• Understanding of contractual, regulatory, and state requirements.
• Capability to educate on benefits and available resources.
• Residency in Arizona is required.
