Care Coordinator

This is a fully remote position, open to applicants in Alabama, +13 more states.

📋 Description

• Oversee a designated panel of 400-450 patients and perform necessary monthly outreach to guarantee the continuity of care.

• Make 50-60 outbound calls to patients each day, offering care plan support and health coaching tailored to their individual needs.

• Uphold call quality standards by consistently engaging and communicating professionally in all patient interactions.

• Work collaboratively with clinical teams, providers, and caregivers to pinpoint and address both clinical and social needs.

• Assist in achieving care coordination objectives by adhering to scheduled monthly outreach goals.

• Act as an advocate for patients by attentively listening to their concerns, investigating issues, and conveying solutions to the patient’s care circle.

• Log all patient interactions in real-time using the care coordination platform.

• Ensure HIPAA compliance in all communications and documentation regarding patients.

⛳️ Requirements

• Must possess a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or an equivalent state-issued MA-C).

• OR Certified Pharmacy Technician (CPhT).

• At least 2 years of clinical experience in primary care, hospitals, or specialized healthcare settings.

• Familiarity with Electronic Medical Records (EMRs) and strong skills in computer navigation.

• Proficient in various technology platforms and capable of navigating complex systems.

• HIPAA-compliant home office setup with a minimum internet speed of 50 Mbps download and 5 Mbps upload.

• Must be legally authorized to work in the United States.
