This is a fully remote position, open to applicants in Virginia.
• Evaluate and assess the needs and requirements of members to ensure they achieve or maintain optimal wellness.
• Provide guidance to members and their families in accessing appropriate care and community resources.
• Conduct field visits to meet members at their homes, nursing facilities, or any preferred locations.
• Assess member goals, needs, barriers, resources, and priorities through both telephonic and in-person interactions.
• Create and adjust Individual Service Plans while involving relevant care team members.
• Assist members with navigating long-term services and addressing environmental and social needs.
• Coordinate access to long-term services, social support, housing, educational, and additional services.
• Recommend Home and Community-Based Services to promote member independence.
• Facilitate communication between payer sources, providers, and Interdisciplinary Care Teams.
• Educate members on how to maintain their Medicaid eligibility.
• Aid in entering annual Level of Care assessments into the state portal.
• Build rapport with nursing facility staff and advocate for the care of members.
• Evaluate members' desires and capabilities to reintegrate into the community.
• Prevent redundant assessments by utilizing available member information.
• Must be a resident of the Central region of Virginia, including Franklin City, Southampton, Sussex, Dinwiddie, or neighboring counties.
• Bachelor’s degree in a health or human services discipline OR a valid LPN license in the Commonwealth of Virginia without any disciplinary actions.
• A minimum of two years of experience in healthcare and/or case management.
• At least one year of experience working directly with individuals who meet Cardinal Care Priority Population criteria.
• Intermediate to advanced proficiency in computer skills.
• Familiarity with Microsoft Word, Excel, and Outlook.
• Proficient in using electronic information applications and software, including electronic medical records.
• Outstanding oral and written communication as well as interpersonal skills.
• Strong critical thinking, organizational, and problem-solving abilities.
• Completion of tuberculosis screening is required if selected.
• Willingness to travel to regional facilities and homes for face-to-face assessments and interactions.
• A valid state driver’s license is necessary.
• Personal vehicle liability insurance must meet state minimum requirements or Humana's specified limits, whichever is greater.
• Preferred: experience in nursing home diversion or long-term care case management.
• Preferred: familiarity with Medicare and Medicaid recipients.
• Preferred: experience in electronic case note documentation and various computer applications/systems.
• Preferred: background in working with complex health populations.
• Preferred: experience in health promotion, coaching, and wellness.
• Preferred: knowledge of Waiver Programs.
• Preferred: understanding of community health and social service agencies as well as community resources.
• Preferred: bilingual or multilingual proficiency, including English/Spanish, Arabic, Vietnamese, Amharic, Urdu, or other languages; must be able to speak, read, and write both languages fluently without assistance.
• Must be available to work Monday through Friday, from 8:00 AM to 5:00 PM Eastern Standard Time.
• Home internet connection must support a minimum of 25 Mbps download and 10 Mbps upload speeds.
• Must work from a designated area without ongoing interruptions to protect PHI/HIPAA information.
• Any associate communicating with members in a language other than English must complete an ILR language proficiency assessment.
