This is a fully remote position, open to applicants in Illinois.
• Handle incoming calls for refill requests.
• Assess the status of refills and process medication renewal requests.
• Initiate outbound calls to pharmacies on behalf of patients when necessary.
• Manage a daily call volume of 40-50 inbound and/or 60-80 outbound calls.
• Collaborate with center care teams to address identified barriers that hinder patients from obtaining refills.
• Use an electronic medical record platform to send requests and updates to field care teams.
• Accurately document information in the relevant electronic systems.
• Record and log all patient requests before concluding the call.
• Monitor the CareReq queue to ensure all open requisitions are completed.
• Arrange transportation for center visits.
• Schedule patient appointments that are off-cadence or specifically requested.
• Utilize the triage nurse line with warm transfers as needed for urgent healthcare issues.
• Process prescription requests in accordance with current standards and policies.
• Engage with patients in a distraction-free environment that supports a secure home office, ensuring the protection of patient information.
• Achieve and surpass departmental metrics while staying updated on any changes to these metrics.
• At least 1 year of experience in retail pharmacy is mandatory.
• Active Pharmacy Technician License in the residing state is required.
• Certification as a Pharmacy Technician (CPhT) is essential.
• A high school diploma or equivalent is necessary.
• Bilingual in Spanish is preferred, with medical Spanish being a plus.
• Strong familiarity with MS Office and Google Suite is preferred.
• Ability to perform daily tasks with minimal supervision.
• Proven track record of delivering exceptional customer service.
• Provide confident and professional service to patients of Oak Street Health and internal partners.
