This is a fully remote position, open to applicants in Kentucky, +2 more states.
• Accountable for engaging in the delivery of patient education and disease management initiatives.
• Conducting health coaching for members across various lines of service.
• Performs behavioral or clinical evaluations to determine individual member knowledge, skills, and behavioral requirements.
• Identifies and/or organizes specific health coaching plans to meet objectives and goals established during assessments.
• Collaborates with providers and other health professionals to synchronize the health coaching plan for the member.
• Implements and/or coordinates coaching and/or care strategies by educating members on their clinical requirements.
• Facilitates referrals to health professionals for behavioral health concerns.
• Utilizes motivational interviewing techniques to promote health behavior changes.
• Assesses and monitors the effectiveness of interventions and/or health coaching plans, making adjustments as necessary.
• Guides members to facilities, community resources, and the appropriate provider/network.
• Refers members to catastrophic case management services.
• An Associate's degree in nursing is required.
• A minimum of 2 years of condition-specific clinical or home health/discharge planning experience is necessary.
• A current unrestricted RN license in the applicable state(s) is required.
• An RN compact license is highly preferred.
• A Bachelor's degree in nursing is preferred.
• Previous case management experience is preferred.
• Bilingual proficiency in Spanish is highly preferred.
• Experience working with members who have chronic conditions is highly preferred.
• Experience in home health settings is preferred.
