This is a fully remote position, open to applicants in Tennessee.
• Evaluates, plans, and implements care strategies tailored to each member, focusing on the most suitable and least restrictive level of care.
• Works collaboratively with members, families, and healthcare professionals to create a personalized care plan.
• Facilitates individual counseling sessions and group therapy for adolescents and adults.
• Recognizes and initiates referrals to social service programs, including financial, psycho-social, community, and state support services.
• Advocates for members and their families as necessary to ensure that the patient's needs and preferences are thoroughly represented and supported by the healthcare team.
• Employs approved clinical criteria to evaluate and ascertain the appropriate level of care for members.
• Records all member assessments, care plans, and referrals made.
• Accountable for meeting established goals and Key Performance Indicators (KPIs).
• Engages with the Substance Use Disorder Program model, taking responsibility for outcome-based care.
• Participates in interdisciplinary team meetings and utilization management discussions, providing information to facilitate safe transitions of care.
• Encourages responsible and ethical management of company resources.
• Maintains exemplary punctuality and attendance during work hours.
• LCSW, LMFT, LPC licensure in Tennessee.
• Preferred experience working with the substance abuse population.
• Background in maternal substance abuse is preferred.
• Experience working with both adults and adolescents.
• Familiarity with Medicare and Medicaid procedures.
• Experience with remote work is desirable.
