Senior Director, Clinical Programs – Clinical Transformation

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Oversee the execution of clinical performance and care models within designated care teams and markets.

• Ensure consistency in clinical decision-making, escalation procedures, documentation, and delivery of evidence-based care.

• Train and enhance the skills of Nurse Practitioners (NPs) and care team members in physiology, pathology, and care guidelines.

• Integrate protocols, care pathways, and best practices into everyday operations.

• Identify and minimize unnecessary variations in care delivery.

• Provide both direct and indirect leadership to clinicians and interdisciplinary team members.

• Mentor teams on complex clinical cases and management of high-risk patients.

• Assist in onboarding, developing, managing performance, and fostering clinician growth.

• Enhance workflows, role clarity, transitions, and care team processes.

• Leverage performance data, clinical insights, and feedback from frontline staff to refine care processes.

• Support panel management, risk stratification, care coordination, and follow-up after discharge.

• Offer clinical insights into decision-support tools, documentation workflows, registries, and technology-enabled care processes.

• Collaborate with product, analytics, informatics, and operational teams to enhance visibility into patient risk, care gaps, and clinical performance.

• Analyze cases of high-utilizer patients to identify opportunities for improving NP-delivered care.

• Take ownership of clinical performance across assigned care teams and markets.

• Propel enhancements in quality, patient outcomes, team performance, and execution of care models.

• Act as a reliable partner to operational, nursing, and enterprise clinical leaders.

• Lead by influence within a highly matrixed environment, promoting a patient-centered and performance-driven culture.

⛳️ Requirements

• MD, DO, NP, or PA credentials are mandatory.

• An active and unrestricted clinical license is required.

• Extensive clinical experience in managing patients with complex chronic diseases is essential.

• Preferred experience includes kidney care, value-based care, primary care, hospital medicine, or population health.

• Proven leadership skills in people management, including coaching, team development, and performance oversight.

• Experience leading interdisciplinary clinical teams in a matrixed setting is required.

• Strong background in value-based care models and execution of care models is necessary.

• Ability to utilize clinical judgment, data, and influence to elevate outcomes on a large scale.

• Exceptional communication, collaboration, and change leadership abilities are vital.

• Preferred experience in kidney care, CKD/ESKD care models, or integrated care for patients with multiple chronic conditions.

• Experience in leading clinicians and managers across multi-site or large-scale care delivery models is advantageous.

• Familiarity with population health strategy, clinical operations, and transformation of care models is preferred.

• Experience in designing or implementing clinical protocols, workflows, or decision-support tools is desirable.
