This is a fully remote position, open to applicants in Canada.
• Assess and determine necessary rehabilitation evaluations to enhance the Participant’s capacity for improvement, clarifying rehabilitation needs and coordinating as necessary.
• Ensure that services and plans are based on individual needs, oriented towards goals, and in alignment with program mandates aimed at improving the six functional domains of well-being while eliminating barriers to re-establishment, ensuring safety, cost-effectiveness, and high quality.
• Create comprehensive Rehabilitation Plans for program Participants in collaboration with the Participant, their family or caregiver, Rehabilitation Service Professionals (RSPs), and the Veterans Affairs Canada (VAC) Case Manager.
• Serve as the service integrator, equally collaborating with medical, psycho-social, and vocational RSPs responsible for delivering services to participants.
• Empower Participants to engage in problem-solving by exploring community-based interventions and resources to reach their desired outcomes.
• Identify barriers or risk factors that may lead to suboptimal outcomes and proactively develop and implement effective strategies.
• A bachelor's degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field.
• A recognized health professional designation; or Certified Vocational Rehabilitation Professional (CVRP or CVRP-F).
• At least 3 years of experience directly providing medical, psycho-social, or vocational rehabilitation services.
• Experience assisting individuals living with mental or physical health challenges that create barriers in daily life.
• Employment is contingent upon obtaining federal security clearance – Reliability Status.
• Willingness to travel across the region as required.
• Bilingualism (French / English) is regarded as an asset.
• Experience collaborating with employers, community resources, and training institutions.
