This is a fully remote position, open to applicants in Missouri.
• Provide exceptional, patient-centered Medical Nutrition Therapy (MNT) for individuals facing chronic and complex health issues such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans that align with each patient's needs, preferences, and objectives.
• Craft nutrition prescriptions that are specifically tailored to clinical diagnoses and foster sustainable behavior change.
• Document all patient interactions in the electronic health record (EHR) accurately and in a timely manner.
• Work collaboratively with healthcare teams, including physicians and health coaches, to enhance coordinated patient care.
• Engage in the development of content for educational purposes, patient engagement, and outreach initiatives.
• Participate in ongoing quality improvement efforts and innovations in care delivery.
• Utilize the latest nutrition research and evidence-based practices in the provision of patient care.
• Maintain all necessary professional credentials, licenses, and engage in continuous education.
• Take part in cross-functional meetings and contribute to strategic projects as appropriate.
• Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration, in good standing AND a Master's Degree in Nutrition or a related field preferred; OR a Bachelor’s Degree with RD/RDN status achieved before the Master's degree requirement takes effect (January 1, 2024).
• Must be licensed in the state of Missouri.
