PRN Nurse Practitioner – Per Visit Telehealth

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform thorough in-home health risk evaluations to recognize all active and chronic health conditions, as well as assess any physical, mental, and social needs at the time of the visit.

• Collect patient history, conduct examinations, and establish diagnoses.

• Provide written documentation of patient visits in accordance with NCQA standards.

• Measure patient vital signs as required.

• Initiate case management referrals and communicate with the primary care physician as necessary.

• Interact with patients, caregivers, agency nurses, other healthcare providers, and vendors as needed to ensure accurate diagnoses.

• Carry out all clinical responsibilities while adhering to OSHA Universal Precautions.

• Uphold patient confidentiality at all times.

• Attend mandatory meetings and in-services and engage in committees as requested.

• Take part in professional development opportunities and maintain necessary professional licenses and affiliations.

⛳️ Requirements

• Active and unrestricted nurse practitioner license for practice in designated coverage states.

• Board certification from one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).

• Current BLS Certification.

• Active enrollment in Medicare/Medicaid programs.

• Must possess a valid driver’s license and maintain a good driving record.

• Exceptional EHR skills.

• Experience or training in geriatrics (preferred).

• Ability to work effectively in a team and build strong working relationships with patients, medical staff, and the public (preferred).
