This is a fully remote position, open to applicants in Maryland.
• Perform detailed virtual assessments and follow-up evaluations for outpatient populations.
• Create and execute personalized care plans in accordance with physician directives and hospital guidelines.
• Provide and modify therapeutic interventions based on patient development and clinical requirements through telehealth.
• Work in conjunction with physicians, case managers, and other interdisciplinary team members to ensure coordinated care.
• Prepare patients for a smooth transition to in-person care when suitable.
• Facilitate discharge planning, patient education, and recommendations for community resources.
• Ensure adherence to HIPAA regulations and the proper execution of telehealth consent procedures.
• Assist patients with basic technical issues to enhance participation in virtual sessions.
• Uphold ethical clinical standards and engage in continuous professional development.
• Graduate from an accredited Physical Therapy program (MPT or DPT required).
• Hold a current Physical Therapist license (multi-state licensure preferred).
• Possess 1–2 years of clinical experience in outpatient or hospital settings.
• Current BLS/CPR certification required.
• Ability to perform all essential job functions following an initial three-month onboarding period.
• Strong interpersonal, patient engagement, and analytical abilities.
• Capability to work independently in a virtual care setting.
• Proficient with telehealth platforms and technology.
