• Oversee the coordination, scheduling, and assignment of palliative patients to healthcare providers.
• Perform follow-up communications with patients and their families to gather feedback regarding their responses to the treatment plans set by the assigned provider.
• Act as the primary contact, advocate, and source of information for patients, care teams, family members/caregivers, payers, and community resources.
• Handle referrals to the necessary agencies as needed, facilitating the client’s achievement of the objectives outlined in their Care Plan.
• Support patients in troubleshooting potential issues connected to the healthcare system, as well as tackling financial or social barriers (e.g., arranging for interpreters, transportation services, or prescription assistance when appropriate).
• At least 2 (two) years of experience in Palliative, Hospice, or Home Care is essential.
• A Bachelor’s degree in a health-related discipline is preferred.
• Experience with medical records is preferred.
• Strong preference for candidates with HCHB experience.
• Demonstrated data analysis abilities with prior experience using an electronic health record.
