LPN Personal Care Coordinator I

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct monthly care management discussions via outbound calls with assigned patients to address their healthcare requirements.

• Relay information about available resources and services to patients throughout their healthcare journey.

• Assess patient-specific issues, objectives, and interventions aimed at fulfilling the individual needs of the patient through action-oriented and timely goals.

• Ensure compliance with charting standards and maintain accurate documentation, including but not limited to medical history, medications, immunizations, allergies, surgical history, and family background.

• Exhibit sensitivity to a patient’s unique situation that may require adjustments to care plans, goals, and preferences, while recognizing potential obstacles to care and services.

• Deliver relevant health education to patients.

• Forward patient concerns to the triage nursing team. Performance metrics include maintaining appropriate utilization, achieving scheduled call percentages, quality scores, and adhering to scheduled hours while ensuring proper attendance.

• Valid and active IL license along with an active multistate license.

• Proficiency in adhering to established clinical protocols and procedures.

• Ability to adapt and switch tasks efficiently.
