This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Create a housing strategy aimed at supporting Horizon membership, particularly for MLTSS/SNP/Medicaid.

• Identify and oversee resources available for housing options.

• Educate all pertinent staff, including Care Management, Member Advocates, and Utilization Management, about housing options and resources.

• Assist in finding and placing members in suitable housing, facilitating their transitions into community settings.

• Prepare reports to comply with state requirements, encompassing progress towards goals and quality monitoring.

• Serve as a liaison with DMAHS staff or their designee to obtain training and capacity-building support.

• Collaborate closely with relevant public and private housing resources and stakeholders, including but not limited to HUD subsidized housing, the Department of Community Affairs (DCA), New Jersey Housing and Mortgage Finance Agency (NJ HMFA) housing program voucher programs, public housing authorities, realtors, and online housing locator tools.


⛳️ Requirements

• A High School Diploma or GED is required.

• A minimum of three (3) years of full-time experience in aiding vulnerable populations (such as the homeless, elderly, and individuals with disabilities) to obtain accessible and affordable housing is necessary.

• At least two (2) years of work experience with a public agency or related experience in a social service or community service environment is required.

• Familiarity with relevant public and private housing resources and stakeholders, including but not limited to HUD subsidized housing, is essential.

• Knowledge of Medicare/Medicaid and Managed Long Term Services and Supports (MLTSS) is required.

• Strong interpersonal and advocacy skills are necessary.

• Proficient verbal and written communication skills are required.

• Good PC skills and the ability to use Microsoft Office applications (Excel, Access, Word) are necessary.

• Strong problem-solving, investigative, mediation, and conflict resolution skills are required.

• Proficiency in reporting and oral presentation skills is necessary.

• Strong organizational skills and the ability to work with minimal daily supervision are required.

• A solid background in processing prior authorizations and demonstrated expertise in navigating various systems is essential.
