This is a fully remote position, open to applicants in Washington.
• Collaborates with and assists members suffering from various chronic conditions in reaching an optimal health status.
• Aids, encourages, and supports self-management objectives and interventions through education, coaching, and support for members with chronic conditions, particularly focusing on diabetes.
• Engages in collaboration with the member, healthcare providers, and internal team members.
• Delivers disease-specific care management interventions aimed at fostering self-management education and coaching for members participating in Diabetes Management programs.
• Responsible for evaluating members, which includes identifying and coordinating access to suitable providers, levels of care, and treatment options.
• Records all member goals, clinical data, outcomes, interactions, assessments, care plans, interventions, and other relevant administrative information in the organization’s clinical database system.
• Identifies barriers to care and collaborates with members and the healthcare team to address those issues.
• Designs and implements individualized care plans that take into account physical, behavioral, cultural, psychosocial, spiritual, age-specific, and educational requirements.
• Coordinates and communicates care activities to ensure continuity and effectively engages community resources as necessary.
• A bachelor's degree in a relevant field or an equivalent combination of education and highly relevant experience is required.
• Current, unrestricted license in the state of Washington as an LPN, RN, RD, or other relevant licensure is preferred.
• Relevant certification such as Health Coach or Certified Diabetes Care and Education Specialist (CDCES) is preferred.
• A minimum of two (2) years of experience in disease management, diabetes education, or health coaching is preferred.
• Previous experience in Managed Care, Medicare, and/or Medicaid is preferred.
• Experience with Motivational Interviewing is preferred.
