Clinical Reviewer

This is a fully remote position, open to applicants in Philippines.

📋 Description

• **Account-specific duties and responsibilities:**

• - Conduct thorough reviews and audits of patient charts and clinical documentation.

• - Confirm that ICD-10 diagnosis codes accurately reflect the documented treatments and services.

• - Assess patient care plans to ensure they are consistent with diagnoses, prescribed treatments, and recorded services.

• - Create structured audit reports comprising over 140 audit questions for each patient chart.

• - Maintain a high level of accuracy, consistency, and adherence to documentation standards during chart reviews.

• - Achieve productivity goals by completing patient chart audits within an average duration of approximately 30 minutes per chart.

• - Identify and flag discrepancies, inconsistencies, or documentation gaps through the reporting process.

• - Adhere to established workflows, auditing procedures, and quality standards for chart review tasks.

⛳️ Requirements

• - Must possess an active Philippine Registered Nurse (PHRN) license.

• - Knowledge of ICD-10 diagnosis codes is essential.

• - Experience in reviewing or reconciling clinical documentation, treatments, and care plans.

• - Familiarity with EMR tools (Kantime) is required.

• - Exceptional attention to detail and strong analytical skills.

• - Proficient in Microsoft applications and web-based forms.

• - Good verbal and written communication skills in English.

• - Must reside within the Greater Manila Area (GMA).

• - Willingness to work night shifts.
